RN Case Manager – Hospital
Marshfield Medical Center, Marshfield, cares for patients from throughout central, northern and western Wisconsin. Marshfield Medical Center offers primary, secondary, tertiary and Level II Trauma Center hospital services provided by Marshfield Clinic specialists. The facility hosts nearly 40 specialties including cancer, heart and orthopedics care, as well as a 24-bed Children's Hospital equipped with the latest technology and highly trained hospitalists and nurses providing nearly 30 pediatric specialty services, including: child abuse-neglect, gastroenterology, pediatric intensive care, neonatology, and surgery. Access to a Level II Trauma Center, ensures 24-hour availability of anesthesiologists, dedicated medical equipment, operating room facilities, physician specialists, surgeons, and trauma nurses.
The RN Case Manager serves as part of the collaborative care team, addressing the patient’s plan of care and expected outcomes with special attention being given to progression towards discharge.The RN Case Manager serves as a resource to the care team with regards to discharge planning and provides review of medical utilization to third party payers to obtain certification for the hospital stay.
Full-time, primarily Monday – Friday day shifts with weekend rotation approximately every 12th weekend.
Requires:
Diploma or degree in nursing; Bachelor’s degree in nursing preferred.
Three years of experience in a medical facility; Discharge Planning, Utilization Review or Case Management experience in a clinic/hospital setting preferred.
Current State of Wisconsin Registered Nurse License or Nurse Compact License.
Additional department-specific certifications, licenses, or memberships may be required (varies by department/location).
